Keeping Youth Safe, Healthy and Connected

Our CMOs serve as the single point of organizational accountability to support need-based intervention and achieve desired outcomes for youth with behavioral health, developmental, or substance use disorders whose ties to home and community are jeopardized.

We work with schools, the Juvenile Justice System, and mental health and behavioral health providers to create plans that will help youth succeed in their home, school, and community. 

Our Approach

We aim to build a comprehensive system that offers youth support from all angles, including natural supports, informal community supports, and formal service supports. Your Care Manager will be there to facilitate the Wraparound Model of Care as well as help you establish a Child Family Team that may include members of your family, friends, community members, and various treatment providers.

Your Care Manager will work with the Child Family Team to develop an Individual Service Plan (ISP) that capitalizes on the strengths of the team and ultimately helps your family meet its goals and overall vision. Our goal is to establish long-term, sustainable strategies that will help kids manage their difficulties for years to come.
